Job description

Encore Healthcare is seeking a Registered Respiratory Therapist to cover for our full time Respiratory Health Navigators in the Tricities area. The respiratory health navigator acts as the clinical leader for one or multiple Encore's Pulmonary Independence Center and other readmission focused programs located within skilled nursing facilities in the Tri-Cities, TN area. Overseeing and providing care for chronic high acuity respiratory patient receiving short term rehab. Encore is a fast paced startup based in Livingston, TN owned and operated by a group of patient-centric clinicians, Pulmonary Physicians and business experts in the respiratory device, homecare, SNF and cardiopulmonary disease management arenas. Encore contracts with health plans, SNF, hospitals, and accountable care organizations to improve health outcomes and reduce overall healthcare utilization for patients living with certain chronic conditions. The Respiratory Health Navigator would be responsible for:

  • Executing on Encore's model
  • Following Encore’s Mission Statement
  • Overseeing day to day clinical operation within the Pulmonary Independence Center
  • Oversee all education for nursing, rehab, and CNA team members
  • Creating and managing relationships with local hospitals, physician, outpatient pulmonary rehab centers and discharge planners
  • Building a relationship with local RT departments to assist in creating a seamless transition from hospital to home
  • Maintaining the highest level of quality for all programs and services with improving patient outcomes being the driving force behind the program
  • Develop strategies and tactics to ensure desirable outcomes

  • Recruit, manage, and execute projects efficiently

  • Update procedures for all execution activities

  • Assist in refining the process as needed

  • Develop, implement, and assess the quality control process during the production of deliverables.



  • Registered Respiratory Therapist with active TN license
  • Minimum of 2.5 years of clinical experience
  • 1-2 years experience in respiratory focused skilled nursing facility
  • A proven track record of managing staff of different departments to produce deliverables according to schedule

  • Good command of MS Office and G suite products

  • Excellent verbal and written communication skills

Preferred Experience:

  • Experience in Pulmonary Rehab and/or
  • Experience in Navigator, case management or disease management role
  • Business Development, Marketing/Liason work
  • Supervisor, coordinator or management experience 

Other Relevant Experience:
